Patient Eligibility

New patients will be able to schedule their first medical appointment, to attend at a later date, on the same day as their eligibility screening.

 

Patients are re-screened for eligibility yearly based on their initial screening date.

 

Patients Must:

  • Reside in Rockingham County.

  • Have no health insurance.

  • Fall under a certain financial income for your household size.

 

To determine eligibility, one must provide a recent bill, last year’s IRS tax return (1040 with schedule C if filed, or 1040EZ), if a tax return has not been filed the clinic has a form to fill out stating no return was filed). Also bring W-2s, all sources of income for you and your spouse including current pay stubs, and a photo id and the previous month’s bank statements (checking and savings).

Patient Responsibility

 

Please bring all of your medications with you to all of your medical appointments.

 

 

Notify Clinic of Changes:

Please immediately notify our receptionist of changes in phone number, address, job or insurance status for you or any other qualified member of your household.

 

Free Clinic of Rockingham Patient Expectations: 

  • Be cooperative in following the guidelines set forth by The Clinic.

  • Be courteous and appreciated of the volunteers and staff for their gift of time to help you.

  • Be patient realizing that many did not get the opportunity to be served.

  • Be honest and forthright in your dealings with us.

  • Be accepting of the limitations that the Free Clinic experiences.

  • Be kind and courteous to others in the waiting area and / or on clinic grounds.

 

Canceling Appointments:

Be considerate of the services The Clinic is providing.  Cancellations must be made at least 24 hours before the scheduled appointment.
This allows us to give your appointment to someone else who needs it.  Be responsible.  If you say you’re coming, COME.

  

The Clinic’s “No Show” Policy is as follows: 

  • 1st No Show – Warning

  • 2nd No Show – 30 day suspension from all services (including prescriptions)

  • 3rd No Show – One (1) year suspension of patient privileges.

 

Hours of Operation: 

Monday thru Thursday:   8:00AM – 4:00PM

Appointments: 

Call on Monday thru Thursday from 9:00AM – 4:00PM.  (336.349.3220 ext. 300)

All appointments are made on a first come first serve basis.

***Walk-ins are not accepted***

 

 

Closed or Full: 

If the clinic is closed or full, seek care from the health department, an urgent care facility, or the hospital emergency room (for emergencies only.)

 

Closings: 

The Clinic will be closed holidays, during bad weather, when there are not enough volunteers for staffing, and as otherwise necessary.  In cases of inclement weather, please visit our Facebook page for up-to-date information.
